What is Skilled Nursing?Today's skilled nursing facilities place emphasis on rehabilitation; getting a person back into a lesser level of care or back into their own home. Very few people actually need to live their later years in nursing homes. Instead, nursing homes are more commonly becoming places where recuperation and therapies are offered.

Transitional Care Units

Hospitals may offer a wing or a floor of transitional care. Licensed as skilled nursing, these units offer immediate transition from an acute care setting to rehabilitation, or short-term stay settings. Their focus is on helping a person return to their previous living arrangement prior to hospitalization.

In some instances, older adults have illnesses which are chronic or cause frailty, but they do not require extended or long-term care. In such instances, older adults may seek care in alternative settings such as Assisted Living Communities, Board and Care private homes, or in-home care can be arranged in a person’s home. Some individuals do require a permanent stay in a nursing home setting, particularly if a condition causes medical instability or care needs that can only be met by continual licensed, skilled nursing care. Nursing home care may be needed.

Rehabilitation Facilities

Some nursing homes offer little or no long-term care accommodations. Instead, they may offer only Medicare or private insurance services for people recovering from illness, an accident or surgery. The intent is to discharge a person from a rehabilitation facility back to a private home, retirement or assisted living or into a private care home.
